About the Role:

We are seeking a highly skilled Psychosocial Support Staff Member to join our team at AGAPLESION in Mitteldeutschland.

Key Responsibilities:

  • Ensure seamless operation of daily activities and provide professional guidance to individuals and their families.
  • Facilitate transitions to appropriate palliative and hospice care, ensuring a smooth and supportive experience.
  • Document counseling services and maintain accurate records.
  • Support quality policies and the facility's mission, promoting a culture of excellence.
  • Collaborate with leadership and maintain effective communication networks.

Requirements:

  • Background in healthcare or social work, with training in Palliative Care.
  • Experience in Palliative and Hospice care, with a strong understanding of the complexities involved.
  • Ability to work collaboratively, take responsibility, and demonstrate effective communication and assertiveness.
  • Strong team player with a willingness to participate in supervision and peer counseling.

Key Tasks:

  • Assist guests and their families, providing emotional support and guidance.
  • Conduct consultations and admissions, ensuring a thorough understanding of each individual's needs.
  • Visit hospitals to discuss hospice care options, promoting informed decision-making.
  • Coordinate costs with insurance and social services, ensuring seamless care transitions.
  • Manage contracts and admissions, maintaining accurate records and documentation.
  • Develop future support services, identifying areas for improvement and growth.
  • Participate in meetings and trainings, staying up-to-date on best practices and industry developments.
  • Support public relations efforts, promoting the facility's mission and values.
  • Maintain relationships with stakeholders, fostering a culture of collaboration and respect.
  • Handle administrative tasks, ensuring efficient and effective operations.
