The successful candidate will work in a dynamic and supportive environment, providing high-quality care and support to individuals with intellectual disabilities and complex needs.

About the Role:

• Work in a residential group home for individuals with intellectual disabilities and complex needs
• Develop and implement individualized care plans in collaboration with residents and multidisciplinary teams
• Document care and support provided, identifying areas for improvement and implementing changes
• Provide active and enthusiastic support to residents in their daily lives
• Contribute to a multidisciplinary team, promoting professional development and knowledge sharing

Requirements:

• Completed education in a relevant field, such as social work, care management, or a related discipline
• Ability to take responsibility and be reliable

• Empathetic and respectful approach to working with individuals with complex needs
