PURPOSE AND SCOPE:
The Outpatient Registered Nurse (RN) CAP 2 serves as a vital entry-level role within the Clinical Advancement Program (CAP) for new hires who fulfill the RN CAP 2 qualifications or those advancing from RN CAP 1. This role is accountable for delivering and coordinating proficient clinical care, which encompasses assessment, planning, intervention, and evaluation for a designated patient group. Responsibilities may include delegating appropriate tasks to direct patient care personnel, including but not limited to RNs, LVN/LPNs, and Patient Care Technicians. As an integral member of the End Stage Kidney Disease (ESKD) healthcare team, this position engages in decision-making, educational initiatives, leadership responsibilities, and quality enhancement activities aimed at improving patient care outcomes and operational efficiency.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
All responsibilities are to be executed in alignment with Fresenius Kidney Care policies, procedures, nursing practice standards, and applicable state and federal regulations.
Executes essential functions under the supervision of the Supervisor, with guidance from the Clinical Educator, Preceptor, or in collaboration with fellow Registered Nurses. Conducts ongoing, systematic collection and analysis of patient data before, during, and after hemodialysis treatment for assigned patients, documenting findings in the patient medical record, making necessary adjustments to treatment plans, and notifying the Team Leader, Charge Nurse, Supervisor, or Physician as required. Evaluates, collaborates, and documents the educational needs of patients and families to provide initial and ongoing instruction. Directs and oversees, in partnership with direct and ancillary patient care staff, all aspects of the daily provision of safe and effective hemodialysis therapy for assigned patients. Administers medications as prescribed or in accordance with approved algorithms, documenting appropriate medical justification and effectiveness. Initiates or assists with emergency response measures as necessary. Acts as a resource, leader, coach, mentor, and role model for new and existing employees by exemplifying appropriate behavior, work habits, and attitudes towards patients, colleagues, supervisors, and the organization. Ensures proper laboratory collection, processing, and shipping procedures are followed and reschedules any missed or insufficient laboratory collections. Identifies expected outcomes, documents, and updates nursing assessments and care plans for assigned patients through collaboration with the Interdisciplinary Team. Ensures patient awareness regarding transplant and treatment modality options. Participates in educational and quality improvement initiatives at the facility and area level as directed by the Supervisor. May serve as a Preceptor for new employees. Required to fulfill CAP requirements to maintain or advance within the program. Performs additional duties as assigned by the Supervisor.PHYSICAL DEMANDS AND WORKING CONDITIONS:

This position involves direct patient care that frequently requires heavy lifting, patient mobility assistance, and ambulation support. Equipment aids and/or coworkers may assist as needed. Frequent, prolonged periods of standing are required, and the employee must be capable of bending over. The employee may occasionally need to move, with assistance, machines and equipment weighing up to 200 lbs., and may lift chemical and water solutions weighing up to 30 lbs. as high as 5 feet. The work environment is typical of a healthcare facility, featuring controlled air temperature and moderate noise levels. Exposure to infectious and contagious diseases/materials may occur. Daily tasks include desk work, computer usage, and interaction with patients, facility staff, and physicians. The position may require travel to training sites or other facilities. May be asked to perform essential functions of this position in other locations with similar physical demands and working conditions.SUPERVISION:
Responsible for overseeing Patient Care Technicians, LVN/LPNs, and RNs as a Team Leader or designated Nurse in Charge, upon meeting all requirements: Successful completion of all Fresenius Kidney Care education and training for new employees, and a minimum of 3 months of experience in chronic/acute hemodialysis as an RN.EDUCATION and LICENSURE:
Graduate of an accredited School of Nursing. Current appropriate state licensure. Current or successful completion of CPR BLS Certification. Must meet the practice requirements in the state of employment.EXPERIENCE AND REQUIRED SKILLS:
