About the Role

We are seeking a highly skilled and experienced professional to join our team at Diakonie Himmelsthür as a Mitarbeiter in der Fachstelle Teilhabe. As a key member of our team, you will play a crucial role in supporting our clients and contributing to the success of our organization.

Key Responsibilities
  • Consultation and Support: Provide expert advice and support to our management team on complex matters related to the overall planning process, including participation in overall planning conferences with service providers, preparation of appeals procedures, and moderation of future planning discussions with people with disabilities.
  • Quality Assurance: Review development reports for plausibility, formulate smart goals, and ensure clear expression.
  • Training and Development: Prepare and conduct training sessions on topics such as assistance planning, personal future planning, or HMB-W and HMB-T procedures.
  • Expertise Development: Build expertise on the overall planning process, including need assessment procedures in Lower Saxony (ICF, etc.), and continuously update knowledge and adapt training content.
Requirements
  • Education and Experience: A minimum of three years of completed, relevant education or study in social sciences.
  • Knowledge and Skills: Good knowledge of the HMB-W procedure, ICF-based assistance planning, BTHG, and instruments of future planning.
  • Communication and Presentation: Good written expression and communication and presentation skills.
  • Personal Qualities: Willingness to take on tasks, resilience, commitment, and flexibility.
  • Interpersonal Skills: Openness in dealing with people with assistance needs.
  • Values and Mission: You respect the Christian heritage of Diakonie Himmelsthür and contribute to the goals and values of Diakonie.
