KWA Hanns-Seidel-Haus is seeking a Certified Nursing Professional (m/w/d) for full-time or part-time roles in home care services.

KEY RESPONSIBILITIES:

• Oversee the resident or client-focused care process through effective planning and evaluation of care strategies.
• Deliver care in accordance with contemporary nursing practices and regulatory standards.
• Ensure the satisfaction and well-being of care recipients.
• Contribute to the formulation, execution, and enhancement of the care framework.
• Engage in care assessments and ensure the precision of care evaluations.
• Support MDK evaluations to determine care requirements.
• Assist general practitioners during home visits.
• Manage the documentation system, ensuring accurate communication of information and observations.

CANDIDATE PROFILE:

• Completed qualification as a geriatric nurse or health and nursing professional.
• Understanding of case management principles.
• Experience in planning and managing care processes.
• Proficient in standard IT applications.
• Commitment to ongoing professional development in the field.
• Dependable and client-focused approach with empathy towards residents and their families.
• Strong reliability, teamwork, communication skills, and personal dedication.
• High level of responsibility and adaptability.
• Willingness to work in a rotating shift schedule, including weekends.
