Principal Duties and Responsibilities:
- Assess patients' psychosocial status and collaborate on effective interventions.
- Utilize social work theory, patient education programs, and interventions for optimal treatment outcomes.
- Provide counseling, monitor patient adjustment to dialysis, and facilitate access to community resources.
- Facilitate transplant referrals, educate patients on ESRD-related issues, and ensure patient rights awareness.
- Deliver training on psychosocial topics to clinic staff as needed.
Education and Required Credentials:
- Masters in Social Work
- State-required license
Experience and Skills:
- 0 – 2 years' related experience
Physical Demands and Working Conditions:
- Travel may be required
